This application is provided by Crossroads – The Abbott Vascular Education Network and the Structural Heart team of Abbott Vascular and is intended solely for health care professionals. The content of this website may only be used for non-commercial purposes, such as research, teaching and/or scientific research. Please read the following terms and conditions (“Terms”) carefully as they govern your membership with SHAPE and use of the SHAPE website. By accessing this website you agree to be bound by these Terms, as amended from time to time.
 
 
REGISTRATION & RESPONSIBILITY

This website’s content, database and functionalities, including the SHAPE application (the “SHAPE Website”), are exclusively accessible by health care professionals who have been granted a login and a password by Crossroads – The Abbott Vascular Education Network (“Authorized Users”). Crossroads – The Abbott Vascular Education Network shall be entitled to refuse access to the Crossroads Website to any user without justification and for whatever reason.

The Authorized User of the SHAPE Website is personally responsible for any messages that he or she posts on the SHAPE Website. Crossroads – The Abbott Vascular Education Network is not responsible for any advice, guidance, opinion or other comment made by any Authorized User of the SHAPE Website.

In addition, Crossroads – The Abbott Vascular Education Network is not liable for any loss or damage that could result from or be associated with the use of the SHAPE Website and/or the impossibility of using it, irrespective of whether Crossroads – The Abbott Vascular Education Network had been advised of such loss or damage. The Authorised User is solely liable for any claim lodged by third parties and shall fully indemnify and compensate Crossroads – The Abbott Vascular Education Network in this respect (including legal costs and lawyer fees).

MEMBERSHIP CONDITIONS FOR SHAPE

Your membership of this website is personal to you and you must therefore not share it with any other person. If you have been issued with a user name and/or password, you agree to keep these secure and confidential and you agree to (i) notify us immediately upon becoming aware of any unauthorised disclosure or use of your user name and/or password and (ii) be liable for any loss or damage resulting from such unauthorised use or disclosure.
 
You agree to use this website only in accordance with these Terms. In the event that your unauthorised use of this website results in loss or damage to any person who then brings a claim against us, you agree to indemnify us for all losses and/or damages arising from such claim.

h) not to disclose any personal data which allows identification of patients when submitting and commenting on cases or otherwise;

i) to inform us immediately of any claim or action against you for any use of this website and, on request from us, to immediately cease the act complained of; and

j) to inform us immediately of any changes affecting your registration details. It is your obligation to ensure that your registration details are kept up to date.

CROSSROADS – THE ABBOTT VASCULAR EDUCATION NETWORK DISCLAIMS ALL LIABILITY FOR THE OVERALL CLINICAL ASSESSMENT OF PATIENTS WHOSE CASE IS UPLOADED ON THE SHAPE WEBSITE. ABBOTT VASCULAR PERSONNEL AND ANY OTHER STAFF POSTING TECHNICAL COMMENTS OR ADVICE ON PATIENTS CASES OR VIDEOS MAKE NO GUARANTEE OR WARRANTY ON THE FINAL CLINICAL ASSESSMENT OF THE PATIENTS.

CROSSROADS – THE ABBOTT VASCULAR EDUCATION NETWORK EXPRESSLY DISCLAIMS TO THE FULLEST EXTENT PERMITTED BY LAW ALL EXPRESS, IMPLIED, STATUTORY, AND OTHER WARRANTIES, GUARANTEES, OR REPRESENTATIONS, INCLUDING, WITHOUT LIMITATION, THE WARRANTIES OF FITNESS FOR A PARTICULAR PURPOSE AND NON-INFRINGEMENT OF PROPRIETARY AND INTELLECTUAL PROPERTY RIGHTS.

Crossroads – The Abbott Vascular Education Network may, in its sole discretion, terminate or suspend your membership immediately without notice for any reason, including (without limitation) if you breach any of these Terms. It may also, in its sole discretion and at any time, cease the provision of this website, or any part of it, with or without notice to you. In the event that Crossroads – The Abbott Vascular Education Network ceases to provide this website, you agree that it may immediately de-activate your membership and delete any personal information relating to your membership.
 
PRIVACY POLICY

Crossroads – The Abbott Vascular Education Network is not responsible for any use of personal data on the SHAPE Website other than the above-listed purposes. Authorized Users are solely responsible for use of any personal data they voluntarily post on the SHAPE Website, including data relating to third parties. Crossroads – The Abbott Vascular Education Network provides automatic anonymization of DICOM video files that are uploaded to the SHAPE Website. Authorized Users are reminded that any form of disclosing patients’ personal data on the SHAPE Website is strictly prohibited.  

REPORTING OF ILLEGAL ACTIVITIES

Illegal activities or information on the SHAPE Website may be communicated to Crossroads – The Abbott Vascular Education Network by sending an e-mail to the following e-mail address: crossroads.brussels@av.abbott.com

REPORTING OF ADVERSE REACTIONS

Please note that health care professionals have an obligation to report all adverse reactions to medicinal products in order to keep complete and updated information on the effects of medicinal products. PLEASE ALSO NOTE THAT CROSSROADS – THE ABBOTT VASCULAR EDUCATION NETWORK DOES NOT PROMOTE ANY OFF-LABEL USE OF MEDICAL DEVICES.

INTELLECTUAL PROPERTY

The copyright in this website, including (without limitation) in all documents, files, text, images, graphics, devices and code contained in it and in this website’s general “look and feel”, is owned by Crossroads – The Abbott Vascular Education Network (All Rights Reserved) or its third party licensors. No links to this website may be included in any other website without prior written permission of Crossroads – The Abbott Vascular Education Network.
